Carolina Musculoskeletal Institute welcomes new patients! CMI works to make sure all patients are seen quickly, and treated warmly. CMI physicians’ ability to properly diagnose you depends partly on an understanding of your medical history and current conditions. If you have never been seen by a CMI physician, or if you haven’t seen one of our physicians in over a year, please fill out the patient forms.
For your convenience, you only need to print the set of PDF forms for the problem you are being seen. Please complete the forms and bring them with you along with your insurance card and a photo identification (driver’s license, state issued ID, etc).

Orthopedics is a specialty within medicine concerned with the diagnosis and treatment of diseases and injuries to the bones, joints, ligaments, nerves and muscles. Services that patients receive in CMI’s Orthopedics Division are expansive, ranging from minor cuts and bruises to treatment for broken bones, torn ligaments and joint replacements.
Neurology is a specialty within medicine that deals with disorders of the nervous system, including the diagnosis and treatment of all categories of disease involving the central, peripheral and autonomic nervous systems.
Rheumatology is a specialty within medicine that deals with the treatment of the arthritis and diseases of the joints and connective tissues. Rheumatology is an evolving field of medicine that strives to understand immunology and disorders related to the immune system. Arthritis and rheumatologic disorders comprise over 200 different conditions. Joint pain, swelling or discomfort can be indications of rheumatology conditions.
Podiatry is a specialty within medicine that deals with the diagnosis and treatment of disorders of the foot, ankle and lower leg. With 26 bones in the ankle and foot—one quarter of all the bones in your body—foot health is important for your quality of life and mobility. The CMI Podiatry Division, under the leadership of Dr. Angela Molnar, offers advanced imaging onsite with High Resolution Diagnostic Ultrasound and Digital Imaging with advanced availability for CMI Imaging Services as needed.
CMI is a Certified Durable Medical Equipment supplier per DMERC for Diabetic Therapeutic Shoe Gear and Insoles, Ankle-Foot-Orthoses (AFO’s), Functional Dropfoot braces, Functional Orthotics, Dynamic Assist Braces and other equipment. CMI’s Podiatry Division is an ongoing FDA-approved clinical trials certified site.
CMI’s Pain Management Division deals with treating acute pain issues following surgery, injuries and joint disorders. Dr. David Goltra Jr. as the lead of the division helps alleviate pain through treatments of steroid injections, epidural injections and spinal stimulator trails.
CMI’s Imaging Division provides the ability to quickly X-ray, scan and view your condition without having to wait on an external imaging center or lab. Our in-house radiologist provides highly responsive diagnosis with the convenience of an office setting. Images are produced via traditional beam X-ray, CT scans and MRI machines. CMI owns a 1.5 Telsa MRI unit which is the most powerful unit in Aiken, SC.
To make a proper diagnosis, a physician may order an X-ray, CT scan, MRI, Myelogram, or Arthrogram. These are considered types of imaging tests because they provide enhanced images of the particular body part that the physician is treating.
CMI’s Physical Therapy division is vital to providing patients with the best musculoskeletal quality care because many musculoskeletal diseases are improved by this treatment. Physical Therapy treats disease or injury with physical methods such as massage, cold therapy, heat treatment and exercise. Physical therapy can be used to help someone avoid surgery, recover from surgery or heal an injury. The goals are to help a patient move better, alleviate pain and make daily tasks and movement easier.
