CMI works to make sure all patients are seen quickly, and treated warmly. CMI physicians’ ability to properly diagnose you depends partly on an understanding of your medical history and current conditions. 

For your convenience, you only need to print the set of PDF forms for the problem you are being seen. Please complete the forms and bring them with you along with your insurance card and a photo identification (driver’s license, state issued ID, etc).
